Recently I lost my PAN Card and applied for re-issue of the same online through tin-nsdl website with acknowledgement no. 881030208617064 on January 19th,2013. Online form had Last Name/ Surname field as mandatory, while my name in all my ID Proofs including Passport, Voter ID Card, Driving License etc. is Dipankar only without any last name. As I was not allowed to fill the online form without any last name/ surname, I entered my father's last name "Kumar" as my Surname. When the very first time I had applied for PAN in paper form, I'd to do the same and had kept display name as Dipankar only. I provided PAN Services Unit of NSDL in Pune, the signed PAN reissue application form with copy of Voter ID Card as Proof of Identity and copy of Driving License as Proof of Address. As these docs have my name as "Dipankar" only and the application form had "Dipankar Kumar", TIN Info team is saying there is mismatch in the name and they are keeping this pending. I was wondering as the docs with my name "Dipankar" were previously accepted for first time issue of the PAN Card why they've problems with re-issue. Also, if they want the name on application form should exactly be matching with name in supporting documents they should not make Last Name/ Surname field mandatory instead they should make first name mandatory. Height was when an executive at NSDL call center told me to provide a Name Change Certificate from Central Gazette. I'm in a fix what to do now. Am I missing their point and not raising a valid issue? Shall I really need to provide any additional documents for getting my PAN Card re-issued? I really need to have clear guidelines on the same, which I'm not getting.
