Sub: Request for Cancellation of insurance policy no. 124897 and Certificate of Insurance issued under Max Life Group Credit Life Plan (Product UIN: 104N018V01) Certificate No. PHR024200699990 and Complaint against AXIX Bank Ltd for wrongfully disbursement in Loan A/C No. PHR024200699990 of Rs.109360 through Life Insurance Policy No 124897 of Max Life Insurance

It is respectfully submitted that I have taken a Loan amounting Rs.4000000/- from AXIS Bank but I surprised to see the insurance policy received at my home on dated 22-04-2013 by which it is certified that Mr. Deepak Setia, Loan A/C No. PHR024200699990 has become an insured member who is covered in accordance with the terms and conditions of the Max Life Group Credit Life Insurance Policy No. 124897 issued to Axix Bank Ltd. (the Policy Holder). My Loan A/c is wrongfully disbursed with Rs.109360/- and bank will charge interest on this amount which is not wanted by me.

Sir, I am not interested in this policy. Please cancel it and correct my loan a/c by settling the amount of Rs.109360 in my Loan A/c.
