Dear Sir/Madam,

This is Simon Richard.

I have changed my Legal name.

Old Name : Chidambaram

New Name : Simon Richard

I’ve applied name change request in Tamilnadu Government Gazette on May 2012 and its been published.

With the help of Gazette publication, I’ve processed my name Change in my passport and driving license.

I’ve processed the name change request for PAN card also through online application via NSDL.


Following are the documents I’ve submitted to NSDL

1) Online Application form with self-attested photo
2) Driving License copy as proof of Identity
3) Passport copy as Proof of Address
4) Gazette publication copy.

I received the discrepancy letter from NSDL as attached in Attachment1

I’ve filled the form as per the attachment(Attachment1)
I’ve only checked the option C
and re submitted the application with Gazette publication and requisition letter to process my application

Even though I have provided all the sufficient documents, they have stopped processing my application.

I’ve the attached following document. Please help me to complete this process ASAP.

1) Discrepancy Letter from NSDL (Attachment1)
2) Gazette proof for name change(Attachment2)(Refer Item No 1033)
